Privacy Policy

Introduction

We are bound by the Australian Privacy Principles. These principles set the standards by which we handle personal information collected from our patients, and are based on the Privacy Act 1988.

This privacy policy provides information to you about how your personal health information is collected and used within our practice.

Why do we collect, use, hold and share your personal information?

The purpose for collecting, using, holding and sharing your personal information is to provide you with the best possible care for your health.

​In order to provide quality health care, it is necessary for the practice to maintain a file pertaining to a client’s health. 

​

What personal information do we collect?
  • name, date of birth, address, contact details

  • health information including medical history, medications, allergies

  • notes made during the course of the consultation which are relevant to your reason(s) for visiting

  • results and reports received from other health service providers

Who do we share your personal information with?

We sometimes share your personal health information:

  • with other healthcare providers

  • when it is required or authorised by law (eg court subpoenas)

  • with colleagues and supervisors for research and learning purposes (de-identified, meaning you are not able to be identified from the information given)

Other than in the course of providing health-related services or as otherwise described in this policy, our practice will not share personal information with any third party without your consent.

How do we store and protect your personal information?

We maintain your personal details and clinical health files electronically on Splose, a secure allied health practice management system. Splose adheres to the Australian Privacy Principles and General Data Protection Regulation, using industry-standard AES-256 data encryption. See Splose's privacy policy here

Your right to access your records and to correct your information

You have rights of access to any information we hold concerning you.  Should you wish to access this information, please put your request in writing via email to hello@slowcity.au. You may also request that we correct or update your information at any time.

Dealing with us anonymously

You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so or unless we are requested or authorised by law to only deal with identified individuals.

Consent

When you register for an appointment at our practice, you provide consent for the practitioners and practice staff to access and use your personal information so they can provide you with the best possible healthcare.

Only practitioners or staff who need to see your personal information will have access to it. If we need to use your information for anything else, we will seek additional consent from you to do this.

 

How can you lodge a privacy related complaint, and how will the complaint be handled at our practice?

Should you have a query or complaint in relation to the privacy policies in place at this practice please contact your practitioner who will discuss any concerns you may have. We will then attempt to resolve it within 30 days of receipt of complaint.
